This form must be completed at least 3 days before your wedding date. Failure to submit your selections by this deadline gives me the right to choose all music at my discretion in accordance with liturgical requirements. If submitted within the 3-day window, I cannot guarantee your selections will be honored but will make every reasonable effort to accommodate you. Music selections made due to late or incomplete submissions are final and cannot be changed. The parish and myself cannot be held liable for any dissatisfaction resulting from late or incomplete submissions.
